Become a tournament sponsor for the 2010 SCPI Event!  This is an excellent opportunity to increase your industry exposure while supporting a very worthy charity, the Osteogenesis Imperfecta Foundation (OIF).  Please mark the appropriate support level and attach your check with this form or call our Treasurer at (949) 768-6422 to arrange for invoicing and / or to request special 30-60-90 day payment terms.  Make your pledge now and pay later!

____     PLATINUM LEVEL ($5,000): Sponsors receive 8 golf or tennis registrations, 8 - $50 Golf Course pro shop gift certificates, 100 raffle tickets, a full page ad in the event program, program sponsorship recognition, two tee sponsorship signs for the two golf courses, and a commemorative plaque.  Select one item from each Class A, B, C below. 

____     GOLD LEVEL ($2,500): Sponsors receive 4 golf or tennis registrations, 4 - $50 Golf Course pro shop gift certificates, 50 raffle tickets, a full page ad in the event program, program recognition, two tee sponsorship signs for the two golf courses, and a commemorative plaque.  Select one item from Class A below. 

____     SILVER LEVEL ($1,000): Sponsors receive 2 golf or tennis registrations, a ˝ page ad in the event program, program sponsorship recognition, two tee sponsorship signs for the two golf courses, and a commemorative plaque.  Select one item from Class B below. 

____    BRONZE LEVEL ($500): Sponsors receive 1 golf or tennis registration, a ˝ page ad in the event program, program sponsorship recognition, one tee sponsorship sign for only one of the two golf courses, and a commemorative plaque.   

NOTE.   Complementary dinner registrations may be substituted for golf or tennis at the donor’s request.  Remember that each golf or tennis registration already includes a box lunch, dinner, and beverages.

Golf Shirt Sponsor:   Two golf shirt sponsors, at a $3,000 donation each, may have their company name or logo put on the sleeve of the golf shirt if artwork is provided to us by Sept 28th.   Shirts are provided to committee members and our event volunteers. Each shirt sponsor will receive: 4 golf shirts, a full page program ad, two tee signs, sponsor recognition, a plaque, and 4 comp golf or tennis registrations. Please let us know if you are interested.  

Make sure your payment / commitment reflects the total amount you have selected for the categories above.  For example, if you have selected to sponsor the entire “Lunch”, the total amount is $2,000  (4 x $500).  This arithmetic summation does not apply if you are a Platinum, Gold, or Silver sponsor, selecting your recognition category.
